Your initial exam will last approximately one to two hours depending on your case. We will review your medical and dental history, possibly obtain diagnostic radiographs and perform a comprehensive examination of your teeth and gums. In most cases, during this initial consultation we will be able to provide a diagnosis of your periodontal and/or peri-implant health and propose a treatment plan to improve your dental health.
Because we know your health is important, Dr. Dalessandro spends as much time as necessary with each patient, until you are confident and comfortable with his recommendations. We have always followed strict sterilization procedures as outlined by the Centers for Disease Control and Prevention (CDC) and, in light of COVID-19, we are currently taking additional precautions.
Prior to your appointment, you will be emailed, texted or snail mailed health history and registration forms, depending on your preference. Please fill these out and bring them with you. Please assist us at the time of your first visit to our office by providing the following information:
- Your referral slip (if you have one), health history, and registration forms
- Any X-rays from your referring dentist (if given)
- A list of medications you are currently taking
- Your dental insurance card, if insured
- Your patient history and information forms
Please arrive on time for your consultation so we can verify any information. A parent or guardian must accompany patients under 18 to this consultation appointment.
We require payment for your first visit at the conclusion of your consultation with the doctor. If you have Dental insurance, we will submit a claim to your insurance company on your behalf after services are complete, requesting reimbursement to be directed to you.
